  • Covered Disability - non-occupational injury or illness
  • Eligible Employees - may include, but is not limited to all active full and part-time employees, and / or a specified class or classes of employees as determined by the employer, subject to carrier approval.
  • Benefit Maximum - maybe a flat amount or multiple of earning, and  /or benefit limits may vary within each class as determined by the employer, subject to underwriting and the selected carrier.   
  • Waiting Periods - may apply as determined by the employer prior to receiving benefits usually following 30, 60 or 90 days from the date of hire.
  • Minimum Hours - employees maybe required to work regularly a minimum of 20 to 40 per week as determined by the employer, subject to underwriting approval by the selected carrier.
  • Contributions - Employers may elect to pay all (Non-Contributory), or part of the cost of premiums for any class(s) of employees (Contributory).
  • Definition of Earnings - benefits maybe provided based on a flat amount or multiples of annual earnings that may include based salary, bonus, and / or commissions up to a specified maximum.
  • Guarantee Issue - no medical underwriting and / or related requirements based benefit plans up to pre-determined limits with selected carriers based on the number of covered lives.
  • Benefit Waiting Periods - may vary from zero (0) days for accidents and three (3) days for illnesses, or pregnancies.
  • Covered Payroll - choice of 50%, 60% or 66.2/3% of salary replacement for employer-paid plans
  • Maximum Benefit - amount up to $5,000 per week
  • Maximum Duration - cchoice of 13-, 26- or 52-week benefit duration
  • Performance Guarantee - maybe vailable for groups greater than 1,000 employees
  • Daily Hospital Benefit - may be available on some plans with selected carriers
  • Special Reports - Third Party Sick Pay (FICA), Diasbility Mangement reports and W-2 Preparation maybe available based on selected plan and insured lives.

  • Maternity - covered as any other illness
  • Rate Guaranteed - maybe 12, 24 or 36 months based on selected carrier
  • Supplmental Plans - available in New York, New Jersey, California, Hawaii, and Rhode Island  
  • Leaves of Absence - employees maybe covered up to 12 weeks under FMLA and the employer may elect to cover employees on voluntary leave for periods form 30 to 180 days, subject to underwriting approval by the selected carrier.
